We see patients by appointment only for non-emergency conditions.  Please arrive at least 15 minutes prior to your scheduled appointment time and bring your insurance card.  Any needed co-pay or deductible amounts will be collected at the time of the visit.  We do accept VISA or Mastercard.  One of our patient advocates will confirm your registration and insurance information with you.  Once registration is completed, you will be ready to see the physician.  Our clinical staff will get your chart and measure your vital signs.  Your doctor will then interview and examine you.  Afterwards, any needed procedures, lab work, or x-rays will be scheduled by our scheduling staff.

Final Reminders:

1. Bring a list of your current medications including doses.
2. If you cannot keep your scheduled appointment, please notify us 24 hours prior.
