Patient Information

Thank you for choosing Oak Ridge Gastroenterology Associates for your healthcare needs. We are pleased to be part of your personal healthcare team.

Appointment Scheduling

Appointments for routine office visits can be made by calling us at (865) 483-4366. Please leave your information with our scheduler or her voice mail. We will call you back. For any emergency problem, you should go directly to the Emergency Room at your hospital. We do not provide emergency care in our clinic.

New Patient Registration

On your first visit, you will be asked to fill out registration forms. You may choose to pre-register by downloading, printing and completing the forms below. Just bring them to your first visit. Completing the forms prior to your visit will help speed up the new patient registration process.

Patient Reminders

We see patients by appointment only for non-emergency conditions. We ask that you please arrive 10-15 minutes prior to your scheduled appointment. If you cannot keep your scheduled appointment, please notify us 24 hours prior. Please bring the following:

  • Your current health insurance information – to ensure accurate and appropriate billing of your health care services
  • Your current Photo ID
  • Any medications you are currently taking in their original containers
  • Any needed co-pay or deductible amounts will be collected at the time of the visit. We do accept VISA or MasterCard.

Insurance

Oak Ridge Gastroenterology Associates participates with Medicare and most major HMO and PPO insurance plans. As a courtesy, we will file all claims with your insurance company. If you have questions regarding your insurance coverage, please contact us at (865) 483-4366. Some plans require referral by your primary care provider. If your insurance coverage includes co-pays we will request payment of these at the time of your visit. Be sure to contact us if your insurance coverage changes in any way. Medical Records

We protect your privacy. Only with your signed authorization can a copy of your record be released. Records can be sent at your request to other physicians or hospitals. We charge a copy fee for records for you, insurance companies, attorneys, etc. This fee is for the work involved to prepare your record and is to be paid in advance. We request at least two business days notice to copy records. We make every effort to send requested records as soon as possible.

Billing

Remember, you are responsible to pay your medical bills, regardless of insurance or any other circumstances. Deductibles, co-payments and non-covered services are your responsibility and you will pay these amounts at your office visit. For your convenience we accept VISA, Master Card, and American Express.

If your account becomes past due without a valid reason for payment delay, we will take action to recover the bill. Please do not ignore the statements which you receive. Any time you have a question concerning your account, please call us.

Prescription Refills

Prescriptions and refills are issued during office hours Monday thru Friday. Please allow for up to two business days to complete a refill. Many prescriptions are refilled electronically, please ask your pharmacy to send a request if you need a routine refill. When calling for a prescription or refill, we need your name, phone number, and the name/phone number of your pharmacy. If your doctor determines if a prescription is indicated, an order will be sent to your pharmacy. Our physicians do not routinely prescribe narcotic medications. Our physicians may require that you come for an office visit to evaluate the need for a new medication or a medication refill.

Direct Access: Colonoscopy

Since many patients over age 50 need a colonoscopy for colon cancer screening, we have a direct scheduling option. Our program allows patients who meet criteria to arrange a colonoscopy over the phone. If patients prefer to be seen prior to a colonoscopy, we are happy to see them in our office.

Direct Access: Endoscopy (EGD)

Gastroesophageal reflux (GERD) is an increasingly common condition. Many patients with GERD will need an EGD. We have a direct scheduling option which allows patients who meet our criteria to arrange an EGD over the phone. Again if patients prefer to be seen prior to an EGD, we are happy to see them in our office.
